How to create a public form

Public forms are designed for individuals outside your organization—such as temporary employees or contractors—who need to complete and submit forms. A link or QR code is provided, allowing them to easily access and fill out the form.

  1. From the toolbar, click (Access), then select Public Settings to open the Public Forms window.
  2. Toggle on the Public Form option.
  3. Click + Public Form Links to create the form link of the URL specific to this public form.
  4. Click the + button to map fields on the form to URL parameters, which will pre-fill information on the public form. If a mapped field already has a default value, it will be overridden by the mapped entry.

    For example, if your employees typically perform inspections in North Dakota (ND), you may have set ND as the default state. However, if vendor workers using the public form are more likely to select Montana (MT), you can map the state field with MT in the URL to automatically pre-fill it for them.

  5. A QR code is generated with the data from the URL.
  6. Multiple links and codes can be set up for each form, which is useful if you are prefilling data for a particular location. For example, let’s say you have two cities where you have vendors performing services and are using public forms. You can create one link for City A by selecting City as the Form Field, and A as the Entry, then repeat from step 2 for but this time enter B as the City entry. Users who use the link for City A will see that field populated on their form, and users for City B see the City B on their form.
  7. If you wish, click the Copy icon to copy the link and paste it into another location to provide to the public form users to open the form.
  8. Click the Complete Links tab to determine what public form users will see or go once they submit the form. They can be shown a “landing page” where they’ll see the default message or enter your own message to appear on a “landing page” after a form is submitted.
  9. If you wish, use the Redirect Link field to paste in a link from another location. Form users will then be taken to that location instead of the landing page when the form is submitted.
  10. Click Save to save your entries.
